1. Proof of Immunizations   Pennsylvania Immunization Requirements

Children in ALL grades (K-12) need the following immunizations for attendance:

• 4 doses of tetanus, diphtheria, and acellular pertussis (DTaP) (1 dose on or after 4th birthday)
• 4 doses of polio (IPV) (4th dose on or after 4th birthday and at least 6 months after the previous dose given)
• 2 doses of measles, mumps, and rubella (MMR)
• 3 doses of hepatitis B (Hep B)
• 2 doses of varicella (chickenpox) or evidence of immunity (History of chickenpox or antibody titer)

ADDITIONAL immunization requirements for attendance in 7th and 12th grade:

• 2 doses meningococcal conjugate vaccine (MCV)
  first dose prior to entry into 7th grade
• a second dose is required at age 16 or prior to entry into 12th grade.
• If the student has already received the first dose after age 16, only one dose is required.
• 1 dose of tetanus, diphtheria, acellular pertussis (Tdap)

2. A medical examination upon original entry to school and in grades 6 and 11.

3. A dental examination upon original entry to school and in grades 3 and 7.

Please try to have these examination forms completed during the summer months before your child reaches the designated grades. These grades were selected because they represent critical periods of growth and development. We are recommending that your family doctor and dentist do these examinations since they can best evaluate your child’s health and assist you in obtaining the necessary treatment and corrections.

Note: The dental and physical exams MUST be completed after September 1st of the year prior to your child’s entrance to the school.

It is important that we have an up-to-date record of your child’s health status. This knowledge enables the school staff to help children achieve maximum benefits from their educational opportunities.
